Records that come from other doctors' offices are the property of that physician's office, and only you can give authorization to have those records transferred. You must first sign a release form (available at your referring physician's office) and ask that those records be sent to your new physician.
It can take up to 30 days to transfer records. However, it often only takes a week or two. Please allow enough time between the records transfer request and your appointment date for this to be accomplished.

The preferred method of records transfer is to allow the office to mail or fax the records. To prevent the possibility of any lost or misplaced records, we do not recommend hand carrying records to your appointment unless you have specifically asked the doctor's office.
